When you confirm an event with a publisher or author, it's helpful to have all the terms and contact information available on a one sheet for reference. This is the form used by Quail Ridge Books (which is why you see their staff names at the top -- you can edit or delete these). The form has space for basic publicist contact info, coop amounts and coop contact.

When you pitch an event to a publisher, it is helpful to have a sheet on hand with your store's capacity, capabilities, and past examples of successful events. A resume, of sorts. The Bookworm in Edwards, Colorado provides this example of the pitch sheet they give to prospective partners when planning an event.

This is the contract that Bristol Books in Wilmington, NC used when committing to providing books for offsite events. Terms depended on whether or not the store was expected to staff the event, or just provide merchandise for the event producers to sell on their own. The file is an editable word document, so specific percentages and sales tax figures can be changed to suit your store needs.
